Responsibilities

• Ensure all residents on psychotropic medications are screened and assessed for clinical appropriateness.
• Monitor for adverse reactions and evaluate potential for drug reductions.
• Complete MDS documentation and evaluations.
• Obtain informed consents and develop care plans for residents on psychotropic medications.
• Congruently coordinate psychiatric services.

Requirements

• An unencumbered State of Illinois RN license.
• C.P.R certification.
• One (1) year of RN experience in a Skilled Nursing Facility setting is preferable but not required.

Qualifications

• Strong clinical judgment and attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Skills

• Knowledge of psychotropic medications and their effects.
• Experience with MDS documentation and evaluations.
• Effective problem-solving and decision-making abilities.
