RN (PRN)

Responsibilities

• Perform client assessments and develop individualized care plans.
• Provide case management, supervision, and coordination of nursing services.
• Respond to referral requests and ensure effective home care delivery.
• Operate in accordance with agency policies, professional standards, and the Nurse Practice Act.
• Perform additional duties as assigned by the Director of Clinical Services.

Requirements

• Current RN license in Colorado.
• Minimum of two years of nursing experience in long-term care, dementia care, or acute care; home care experience preferred.
• CPR certification.
• Graduate of an accredited nursing program; Baccalaureate degree preferred.
