RN - Primary Care Nurse Coordinator - Part Time

Role Overview

This position involves serving as an expanded nursing team member to promote preventative care, conduct screenings, coordinate disease management, and support primary care providers. The nurse will assess patient needs, develop individualized care plans, educate patients and families, and facilitate communication among healthcare team members.

Responsibilities

• Perform clinical interventions, including medication administration, IV management, and diagnostic test coordination.
• Assist with triage, manage patient interactions, and identify intervention pathways based on clinical guidelines.
• Coordinate laboratory and diagnostic services; document activities accurately in electronic health records.
• Provide education on disease management, health promotion, and care coordination processes.
• Participate in professional development, quality improvement, and care transition activities.
• Support interdisciplinary collaboration and contribute to program design and evaluation.

Qualifications

• Licensed RN with at least two years of clinical experience, preferably in Med/Surg settings.
• Graduate from a specialty training program in nursing; Bachelor's degree preferred.
• Strong clinical, communication, and patient education skills.
• Ability to collaborate effectively within a team and apply evidence‑based practices.
• Proficiency in documentation and data collection in electronic health systems.
