About the role

This role involves providing patient-centered home care to improve health outcomes and reduce hospital visits. The nurse practices independently within an interdisciplinary team, delivering personalized care plans, assessing patient needs, and adjusting treatments accordingly.

Responsibilities

• Deliver individualized nursing and care management in patients' homes or care facilities.
• Evaluate and respond to evolving patient conditions with appropriate plan modifications.

Qualifications and Skills

• Current RN license and registration in New York State.
• Bachelor’s or Associate’s Degree in Nursing.
• Minimum four years of clinical home care experience, including at least two years full-time and one year as a field staff nurse in home care.
• Ability to manage cases with minimal supervision, assess problems, develop action plans, and evaluate outcomes.
