About the Role

Avantara Lincoln Park is a premier skilled nursing and rehabilitation facility located in Chicago’s Lincoln Park neighborhood. We provide advanced rehabilitation therapies and exceptional nursing care in a vibrant, historic community setting. The Nurse Supervisor R.N. plays a critical role in delivering superior customer service and nursing services to all residents (referred to as "Guests") on the assigned unit and throughout the facility. This position assists the Assistant Director of Nursing (A.D.O.N.) and Director of Nursing (D.O.N.) in planning, developing, and overseeing the unit’s operations to ensure high-quality, 24-hour care.

Responsibilities

  • Assists the D.O.N. and A.D.O.N. with planning, directing, and supervising the activities of the nursing staff.
  • Ensures the Nursing Department complies with federal, state, and local regulations.
  • Develops, implements, and updates department policies and procedures as needed.
  • Assists with staffing to meet resident needs and regulatory requirements.
  • Participates in Federal, State, and Local agency annual surveys.
  • Reviews deficiencies noted by government agencies and assists with implementing corrective actions.
  • Provides guidance, education, counseling, and disciplinary action to staff as needed.
  • Trains and educates staff members on the assigned unit.
  • Ensures adequate staffing at all times, including filling in as a floor nurse and coordinating breaks.
  • Maintains infection control practices and standard precautions.
  • Follows established safety precautions when performing tasks and using equipment.
  • Facilitates nurse-to-nurse admission and discharge reports to/from hospitals and other institutions.
  • Reports hazardous conditions, damaged equipment, accidents/incidents, and supply issues.
  • Maintains the comfort, privacy, and dignity of residents, promoting a caring environment.
  • Ensures each resident receives person-centered care.
  • Responds promptly and courteously to call lights when working in resident care areas.
  • Assists with new admissions, re-admissions, and resident transfers within the unit.
  • Communicates effectively with residents, visitors, families, peers, and supervisors.
  • Attends and participates in departmental meetings and in-services.
  • Participates in Quality Assurance and Performance Improvement (QAPI) efforts, including training, role execution, and communication of outcomes or barriers to leadership.
  • Adheres to compliance plans and participates in compliance trainings.

Supervisory Responsibility

This position directly supervises all nursing staff on the assigned unit, including R.N.’s, L.P.N.’s, and C.N.A.’s.

Physical Demands

  • Regularly required to talk and hear.
  • Very active position requiring standing, walking, bending, kneeling, and stooping.
  • Must be able to lift and move items and patients weighing at least 50 pounds.
  • Requires close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Schedule

This position is part of a 24-hour, 365-day healthcare facility. Rotating shifts, holiday work, and inclement weather coverage are required.

Requirements

  • Graduate of an accredited nursing program.
  • Valid, unencumbered Registered Nurse (R.N.) license in the state of practice.
  • C.P.R. certified.
  • Minimum of 2 years of R.N. experience.
  • Knowledge of Federal, State, and Local healthcare regulations.

Preferred Qualifications

  • 5 years of R.N. experience.
  • Previous experience managing a nursing department or unit.
