Responsibilities

• Triage patients
• Manage prescription renewals
• Aid in patient education regarding medication adherence
• Coordinate Transition of Care Calls
• Provide vaccinations, including flu shots and other injections
• Follow up on test results
• Collaborate with interdisciplinary teams to enhance patient outcomes
• Ensure compliance with competency requirements for specialty areas
• Participate in quality assurance and performance improvement initiatives
• Act as an educator to peers, patients, and their families

Qualifications

• Graduate of an accredited school of professional nursing, BSN preferred
• Current RN licensure in the state of Florida
• American Heart Association BLS Certification for Healthcare Providers
• Minimum of 1 year experience, 2 years preferred

Physical Requirements

• Requires stooping, sitting, standing, and walking for long periods
• Frequent assistance in lifting patients and heavy equipment
• Must work in a stressful environment, make sound decisions, be detail-oriented, alert, and self-motivated
• Ability to push/pull up to 40lbs, carry and lift up to 50lbs

Threshold Requirements

• Annual Joint Commission mandatory education requirements
• In-service and health requirements including attendance at new employee orientation
• TB/PPD Surveillance Program
• Maintenance of required professional licensing and/or certification(s)
