Responsibilities
- Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery
- Develop, review, and revise individualized plans of care based on patient needs
- Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards are met
- Maintain accurate clinical documentation, care plans, and service records
- Communicate professionally with caregivers, clients, case managers, and leadership teams
- Participate in staff meetings, in-services, and continuing education opportunities to support professional growth
Requirements
- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in good standing in the state applying for
- CPR Certification required
- Minimum of one (1) year of nursing experience; Home Care experience preferred
- Knowledge of Medicare/Medicaid, home care benefits, and applicable State regulations preferred
- Strong clinical assessment, communication, and organizational skills
- Excellent computer proficiency and documentation abilities
- Ability to multitask and adapt in a fast-paced healthcare environment
- Professional verbal and written communication skills
- Ability to motivate and lead caregivers in a positive and supportive manner
- Reliable personal transportation is required
- Ability to travel locally, and regionally to support additional field offices as needed
Qualifications
