Responsibilities
- Provides assessment of physical, emotional, socio-economic needs and environmental needs of patients and their families.
- Identifies problems using patient data base, nursing process, and physician’s plan of treatment.
- Develops a plan of care and interprets it to the patient and family regarding the diagnosis and treatment.
- Observes, evaluates, and reports to the physician the patient’s physical and emotional condition, reactions to drugs or treatments.
- Interprets social and physical factors affecting physical care in the patient’s environment.
- Assesses health status through observation, interview, and analysis.
- Counsels families in basic principles of wellness and applies information from related fields such as nutrition, safety, education, mental health, and dental care.
- Performs screening and diagnostic procedures according to approved medical policies, procedures, and competency levels.
- Aids physicians with examinations and treatments in participating clinics.
- Interprets test findings to individuals or families and makes appropriate referrals.
- Obtains laboratory specimens when necessary.
- Gives preventive immunizations or treatments under medical supervision.
- Attends required continuing education courses.
- Participates in Quality Management Program.
- Fulfills all compliance responsibilities related to the position.
- Performs other duties as assigned.
Requirements
- Current RN License in State of Connecticut
- Minimum Experience: Zero to six months
- Desired: BSN
- Experience in a qualified home health agency
Qualifications
- Manual: Significant manual skills/motor coordination & finger dexterity
- Occupational: Significant occupational risk
- Physical Effort: Significant exposure to dirt, odors, noise, human waste, etc.
