Job Duties
- Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing patient or member health status.
- Manages utilization and practice metrics to refine care delivery models.
- Develops systems of care that monitor progress and promote early intervention in acute care situations.
- Designs individualized plans of care and fosters a team approach with patients, families, and healthcare teams.
- Maintains required documentation for all case management activities.
- Collects and utilizes data to adjust treatment plans as needed.
Requirements
- At least two (2) years of RN work experience.
- Certified Case Manager / American Case Management Association (ACMA) within two (2) years of hire date.
- Bachelor's Degree in Nursing (BSN) preferred.
- Strong pediatric nursing experience preferred.
- Case management experience preferred.
- Knowledge of pediatric healthcare laws, regulations, and insurance coverage policies.
- Proficiency in healthcare IT systems and insurance claims processes.
- Ability to work autonomously and as part of a multidisciplinary team.

Education & Experience
- Graduate from a Specialty Training Program-Nursing.
- Bachelor's Degree-Nursing preferred.
- Minimum of 2 years-Nursing experience.
Certification & License
- Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania.
- Certified Case Manager within 2 years - American Case Management Association (ACMA).
