Role Overview: Serve as a patient navigator in the Hematology/Oncology department, coordinating care and acting as a liaison among management, physicians, community organizations, and patients. Key Responsibilities Coordinate treatment efforts, promote quality improvement, and support educational initiatives.Compile and analyze data, track outcomes, and recommend process enhancements.Provide referrals and follow-up support for uninsured or underinsured patients. Qualifications & Skills Registered Nurse licensure from the Indiana Board of Nursing.Preferred BLS Provider certification (American Heart Association or Red Cross).Diploma from an accredited nursing program or equivalent licensure.Oncology Certified Nurse (OCN) and two years of oncology or case management experience are preferred.
