About the Role

A Patient Care Manager leads the in-field clinical team, ensuring high-quality patient care while upholding professional standards, agency policies, and regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements
• Coordinate and monitor patient care delivery
• Supervise clinical staff to ensure consistent, appropriate service
• Maintain clear communication with patients, caregivers, referral sources, and field and office staff
• Drive quality outcomes by meeting department goals and supporting improvement initiatives
• Model professionalism and organizational values in daily work
• Uphold compliance through required training, accurate reporting, and cooperation with audits
• Promote a safe work environment by following established safety protocols
• Support staff growth through coaching and continuing education

Requirements

• Must have a current or reinstatable license in the state
• Previous experience in a home care setting with two years of management or supervisory experience (preferred)
• Knowledge of accepted professional standards and practices, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
