About the Role

The Patient Care Manager oversees and supports clinical staff to deliver compassionate, high-quality home health care. They coordinate services, ensure compliance with professional and regulatory standards, and foster a culture of safety, excellence, and trust.

Responsibilities

• Provide clinical supervision to ensure care aligns with standards and regulations.
• Coordinate patient care by planning and monitoring services throughout the care episode.
• Supervise clinical personnel to ensure consistent and timely service delivery.
• Maintain effective communication with patients, caregivers, and staff.
• Drive quality outcomes by meeting departmental goals and supporting quality improvement.
• Model professionalism and excellence by demonstrating organizational values daily.
• Uphold compliance through completing training, accurate reporting, and cooperating with audits.
• Promote safety by reinforcing safety protocols.
• Develop the team through coaching staff and encouraging continuing education.

Qualifications

• Registered nurse with current licensure.
• Preferred experience in home care management.
• Knowledge of professional standards, Medicare requirements, and regulatory compliance.
