RN - Patient Advisory

Role Overview: The position involves providing telehealth triage and patient care support, acting as the first contact for patients via phone, video, or electronic communication. Responsibilities include: Assessing patient concerns, developing care plans, documenting encounters in EMR, managing medications, educating patients, facilitating care coordination, and communicating with outside facilities. The role requires supporting clinical operations during day and after hours, acting as a Charge RN in rotation, and cross-training across departments to assist with medication refills, triage, and electronic patient communications. Qualifications and Skills: Valid RN license in Illinois (initially Indiana within 1 year), a nursing diploma, and minimum of 2 years of nursing experience. Strong verbal communication, critical thinking, and patient-centric focus are essential. This role combines autonomous practice with adherence to established protocols, emphasizing quality care improvement and team support to ensure patient safety and satisfaction.
