About the Role
This position provides expert palliative care support to patients and families, enhancing primary care capabilities through advanced communication, goals-of-care discussions, and symptom management. You will coordinate care across home, inpatient, hospice, and community settings, support advance care planning, and ensure accurate documentation of care transitions.
Responsibilities
- Lead advanced communication and goals-of-care discussions with patients and families.
- Coordinate palliative care across home, inpatient, hospice, and community partners.
- Support advance care planning and document care transitions accurately.
- Educate staff on serious illness communication and palliative care principles.
- Promote interdisciplinary collaboration and participate in team reviews.
- Maintain strong relationships with patients, families, and providers.
- Ensure compliance with organizational policies and regulatory standards.
Requirements
- Active RN license in the relevant state.
- 3+ years of clinical or hospice experience.
- Current BLS certification or ability to obtain within 30 days.
- Valid driver’s license and reliable transportation.
Preferred Qualifications
- BSN degree.
- Palliative care certification.
- Experience with geriatrics and complex case management.
