Primary Location: Liberty, LA • Department: Liberty PACU • Full-time (0.9 FTE) • 36 weekly hours • Day shift

Responsibilities

  • Apply clinical judgment by synthesizing and interpreting multiple data sources, anticipating problems, and seeking interprofessional collaboration to respond to complex situations.
  • Lead efforts to improve documentation of the plan of care, care delivered, and outcomes of care.
  • Engage patients and families in the plan of care, addressing current and future needs.
  • Collaborate with families, healthcare providers, and community resources to develop comprehensive care plans.
  • Orchestrate caring practices to address patient and family comfort and concerns, promoting a healing environment for all outcomes, including end-of-life care.
  • Integrate age-specific and culturally inclusive concepts into patient care, considering both chronological age and developmental level.
  • Develop care goals and actions in partnership with patients and families using self-management strategies.
  • Modify or develop patient and family education programs, integrating education throughout care delivery and evaluating effectiveness using teach-back methods.
  • Support patients and families in transitioning to home by integrating relevant strategies into the plan of care.
  • Use systems thinking to navigate and negotiate the healthcare system on behalf of patients and families through care coordination.
  • Lead efforts to change systems and processes to support optimal patient outcomes.
  • Maintain an understanding of regulatory requirements and accreditation standards, working with managers to ensure compliance.
  • Monitor and assist others in complying with safety and departmental policies and procedures, ensuring timely fulfillment of certifications and Safety College requirements.
  • Communicate safety and compliance concerns related to CCHMC processes and CMS standards to leadership.
  • Coordinate team care delivery as charge nurse, anticipating and mitigating unexpected circumstances.
  • Demonstrate consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.
  • Advocate for patients, families, and communities, empowering them to represent themselves and tailoring care to their values and needs.
  • Incorporate inclusive resources and alternative therapies into care, growing cultural competence for self and others.
  • Promote and expect collaborative behavior, mentoring others to respect team contributions and leading shared governance structures.
  • Improve or individualize standards and guidelines for patient care, questioning and evaluating current practices based on patient responses, literature, and research.
  • Create practice changes through research utilization and experiential learning, assisting in the development of peer competence.
  • Use self-appraisal, performance review, and peer review for professional development and safe, ethical practice.
  • Precept nursing peers and students, facilitating learning for the healthcare team and community.
  • Lead efforts to identify, share, implement, and evaluate safety best practices with department managers and magnet representatives.

Requirements

  • BSN from an ACEN, CCNE, or NLN CNEA accredited institution or MSN required.
  • 5+ years of related experience.
  • Active Ohio RN License; may require additional state licensure.
  • Active certification; some roles require specialty certification.
