RN - Outpatient Triage Housecalls (No Weekends, Holidays, or Call)
Summary of Job Duties
The Registered Nurse will promote wellness, improve outcomes, and support self-care within a defined patient population, including those in Assisted Living Facilities and Private Homes. The RN will provide patients with chronic conditions with support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in their homes, assisted living, and skilled nursing facilities. Coordinate prescription refill requests and patient education coordination in collaboration with the physician staff.
Qualifications
- Minimum Qualifications: Bachelor’s degree in Nursing, Associate’s degree in Nursing, Diploma RN, or Master’s degree in Nursing. Must be a graduate of an accredited RN nursing program.
- Valid Registered Nursing License
- Minimum of 1 year of Registered Nursing experience; BLS certification within 30 days of hired.
- Basic computer skills.
- Excellent communication skills.
- Strong work ethics and accountability.
Preferred Qualifications
- Charge RN or Leadership experience.
- One (1) year of clinic or ambulatory setting experience.

Additional Information
Responsibilities:
- Triage, assessment, planning, implementation, and evaluation of nursing care and patient education either by telephone or in the patient's home.
- Evaluates the progress of patients toward the attainment of desired outcomes.
- Administers medication following clinic standards and the Nurse Practice Act.
- Verifies and evaluates care delivered to ensure care is consistent with standards and regulatory requirements.
- Caregives coordination and follow-up for patients who receive care in assisted living facilities.
- Coordinates patients’ prescription refill requests and patient education coordination in collaboration with the physician.
- Delivers nursing care according to the standard of care defined by UAMS, ANA, and Standards for specialty patient populations with support and mentoring by other team members;
- Collects and documents assessment data;
- Develops a Plan of Care (POC) per standard;
- Implements POC including medical order and interdisciplinary approach;
- Evaluates the progress of the patient toward the attainment of desired outcomes;
- Provides effective patient/family education/discharge planning;
- Administers medication per standard including drug information to patients/families;
- Acts as a resource/supports others;
- Delivers patient care using the principles of the defined nursing modality;
- Delegates care based on UAMS standards, legal and regulatory requirements, specific job descriptions, and individual levels of education and training;
- Contributes to a positive work environment conducive to clinical education while serving as a resource for student nurses and staff.
Salary Information
Commensurate with education and experience.
