About the role

As an RN - Trach & Vent Care Specialist for Home Care, you will provide expert care to patients requiring tracheostomy and ventilator support in their homes. This role requires strong clinical skills, excellent communication, and the ability to work independently and as part of a team.

Responsibilities

• Assess patient needs and develop individualized care plans
• Administer tracheostomy and ventilator care
• Monitor patient condition and report changes to healthcare providers
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Document all care activities accurately and timely

Requirements

• Bachelor's degree in Nursing from an accredited program
• Current RN license in good standing
• At least 2 years of acute care nursing experience
• Experience with tracheostomy and ventilator care

Qualifications

• Excellent interpersonal and communication skills
• Ability to work independently and manage time effectively
