RN - Oncology - Patient Nurse Navigator - $3K Sign-on Bonus

Compensation

Salary Range: $58.49 - $89.07 / hour
$3K Sign-On Bonus

Responsibilities

• Enhances the patient experience by providing a seamless navigation process from abnormal finding/diagnosis through the cancer continuum.
• Collaborates with physicians and interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating the patient and serving as a point of contact for patients and families.
• Works with patients and families to answer questions, provide emotional support, and resolve any barriers, needs or issues related to the cycle of cancer care.
• Affirms patients' current and future needs, provides patient access to resources internal and external, facilitates distress screenings and patient needs assessments, and makes appropriate referrals.
• Establishes and maintains close collaboration with physicians and the cancer care team to ensure patient care and coordination using oncology nursing knowledge base, experience and expertise.
• Participates in multidisciplinary conferences, providing patient follow-up as needed.
• Provides patient education and symptom management support.
• Participates in community health promotion and awareness programs.

Qualifications

• California active RN license
• Associate of Science Degree in Nursing
• Three (3) years of oncology nursing experience
• Oncology Certified Nurse (OCN) preferred
• Problem-solving, decision-making, and critical thinking skills
• Excellent organizational, written and verbal communication and interpersonal skills
• Self-directed environment and handling multiple priorities
• Knowledge of the American College of Surgeons (ACoS) Commission on Cancer (CoC) standards preferred
• Bilingual in English/Spanish
• Prior experience planning, organizing, and completing individual treatment plans
• Prior experience implementing professional and community-based education programs
• Proficient in Microsoft Office, Electronic Health Records (EHR), and related database programs
• Knowledge of medical terminology, CPT coding, anatomy, histology, hematology and cancer-related diagnostic procedures
• Knowledge of the American College of Surgeons (ACoS) Commission on Cancer (CoC) standards
