Responsibilities

• Respond to a variety of patient needs, from urgent to routine, providing unwavering support in critical moments.
• Provide empathetic and heartfelt assessments: conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
• Maintain accurate documentation: ensure every detail is captured in the patient's home and within agency and regulatory standards.
• Provide support for families: share knowledge and expertise to guide and support families in caring for their terminally ill loved ones.

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Hold current unencumbered license as a Registered Nurse (RN).
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
• Hospice experience a plus.
• Must be computer proficient in typing and various programs, including a background in EMR.
• Possess and maintain current CPR certification if required by state.

Pay

$75,000 - $85,000 per year (Average Pay Range). Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
