Schedule

FT On Call is a 7 on, 7 off position from Fri 4:30pm through Monday 8:30am, then Mon-Thurs night from 4:30pm-8:30am.

Territory

Napa and Solano County.

Responsibilities

• Be the Lifeline: Respond to a variety of patient needs, from urgent to routine, providing unwavering support in critical moments.
• Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
• Maintain Accurate Documentation: Ensure every detail is documented to deliver high-quality care, both in the patient's home and within our agency and regulatory standards.
• Provide Support for Families: Share your knowledge and expertise to guide and support families in caring for their terminally ill loved ones, offering confidence and reassurance during challenging times.

Requirements

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR one (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintain current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Pay

$132,000 - $135,000 per year (Average Salary). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
