RN Nursing Coordinator - Urology Support Services

Description

The Urology team is seeking a full-time Uro-Oncology Nurse Coordinator to support high-quality, patient-centered care. Key responsibilities include coordinating treatment plans, assisting with discharge planning, and providing patient education.

Responsibilities

• Assesses patient status and rounds with an interdisciplinary team to determine care needs.
• Facilitates patient care by communicating with patients, families, and healthcare teams.
• Collaborates with the interdisciplinary team to develop and deliver patient education materials.
• Collects and analyzes data for trends and makes recommendations for improving patient care protocols.
• Coordinates post-hospitalization care and discharge planning, making referrals to other care providers.
• Attends care conferences and interdisciplinary rounds to document care planning and communication.
• Promotes professional development through attending inservices and presentations.

Requirements

• Graduation from a state-accredited school of professional nursing.
• Licensed to practice as a Registered Nurse in the state of North Carolina.
• Five (5) years of professional nursing experience.
