RN Nursing Coordinator - Pediatric Gastroenterology

About the role

The Pediatric Gastroenterology Nurse Coordinator is responsible for coordinating care for pediatric patients with complex gastrointestinal disorders, including inflammatory bowel disease (IBD), feeding disorders, liver disease, disorders of gut-brain interaction, and other conditions. The nurse coordinator provides comprehensive clinical coordination, patient education, and support for complex and chronic conditions, and serves as a key liaison among patients, families, and the multidisciplinary care team.

Responsibilities

• Coordinate care for pediatric GI patients
• Triage calls/messages with patients/families regarding symptoms and management, and draft a response
• Manage patient tracking systems (e.g., follow-up lists, infusion schedules, high-risk patient registries) to ensure timely care
• Serve as the primary point of contact for patients, families, and providers regarding care coordination and clinical needs
• Work with the scheduling team to ensure timely clinic visits, procedures, infusion appointments, hospital admissions, and annual follow-ups
• Coordinate outside testing/procedures
• Provide patient and family education related to diagnoses, treatment plans, medications, diet, and disease management
• Coordinate care for patients receiving subcutaneous biologic therapies or infusions, including monitoring and follow-up
• Collaborate with physicians, APPs, dietitians, and other care team members to ensure comprehensive care delivery
• Assist with prior authorizations, referrals, and coordination of specialty services
• Monitor patient outcomes and address gaps in care, including missed appointments or delayed follow-ups
• Communicate with patients, families, and other care providers
• Maintain accurate documentation in the electronic medical record (Epic competency preferred)
• Support quality improvement initiatives, patient registries, and clinical program development within Pediatric GI
• Participate in divisional meetings, educational sessions, multidisciplinary meetings, case conferences, and care planning discussions
• Assess patient status of a specialized population
• Round with an interdisciplinary health care team to determine patient care needs and coordinate ongoing acute care and long-term needs
• Act as a liaison between the patient, family and health care team to communicate patient care needs including coordination of patient/family care conferences
• Collaborate with health care team to provide information and resources to facilitate patient care
• Provide inservices and formal presentations to nursing, medical and ancillary staff on topics related to the specialized patient population
• Assist staff in the development of patient education materials for the patient population
• Perform patient teaching related to specific topics and patient needs
• Document patient teaching in the medical record in the appropriate sections
• Collect data related to specialized patient population and analyze for trends
• Enter data or coordinate its entry into databases and registries for benchmarking purposes
• Participate in performance improvement activities to promote quality patient care
• Analyze data for trends and make suggestions for revisions to patient care protocols for the patient population
• Monitor outcomes of care for patient population and collaborate with the interdisciplinary health care team on evaluation of outcomes
• Coordinate patient care for post-hospitalization care and discharge planning
• Make referrals to other care providers during acute care episode and for post-discharge care
• Communicate discharge needs to interdisciplinary health care team, patient and family
• Coordinate appointments with clinics, outside referral sources, home care, durable medical equipment providers and other providers to ensure continuity of care for the patient
• Attend care conferences and interdisciplinary care rounds
• Document care planning and communication with health care providers in the medical record
• Promote own professional development through attending inservices and presentations
• Keep current in practice through conferences, reading current literature and research
• Role model professional nursing practice to others through use of the nursing process and professional behaviors

Qualifications

• Licensed Registered Nurse (RN) in the state of North Carolina
• Bachelor of Science in Nursing (BSN) preferred
• Strong organizational, communication, and care coordination skills
• Ability to manage complex patient populations and multiple workflows
• Proficiency with electronic medical records (Epic preferred)

Preferred Qualifications

• Minimum of 2–3 years of pediatric or clinical nursing experience
• Experience in pediatric gastroenterology, hepatology, and nutrition
• Experience coordinating care for infusion therapies and chronic disease management
• Familiarity with patient registries, tracking systems, and population health management
• Certification in pediatric nursing or gastroenterology (preferred but not required)
• Experience in an academic medical center or specialty clinic
