Job Summary

The RN Nurse Navigator reports to the Operations Manager and functions as a member of the multidisciplinary team of care providers. The role involves coordinating appropriate care for patients, providing education, and supporting continuity of care.

Duties And Responsibilities

• Conducting and overseeing patient assessments (telephonically/in person)
• Aiding patients in navigating and selecting the appropriate Urology care pathway
• Assessing patients clinically and offering recommendations based on clinical information
• Coordinating with other staff and support staff to guide patients through the care pathway
• Working collaboratively with other programs to ensure patient preferences and needs are met
• Participating in meetings and contributing to the improvement of patient care
• Remaining current with research and treatment options
• Following up with patients post-discharge to ensure continuity of care
• Serving as an educational resource for staff and participating in ongoing quality improvement

Required Skills And Knowledge

• In-depth knowledge of Surgical and other available treatment options
• Knowledge of community resources
• Working knowledge of office equipment and computer literacy
• Excellent communication skills
• Ability to work efficiently in EPIC EMR system
• Experience in planning and coordinating multi-disciplinary communications
