About the role
The RN Nurse Assessment (MDS) and Quality Coordinator is responsible for conducting and coordinating the MDS Assessment with the appropriate participation of health professionals. Knowledge of care required: Young Adult to Geriatric.
Responsibilities
- Conduct and coordinate MDS Assessments
- Participate in interdisciplinary team meetings
- Ensure accurate documentation of assessments
- Collaborate with healthcare professionals to ensure quality care
Requirements
- Associate's degree in nursing
- Certified Nursing Assistant (CNA)
- Current California Registered Nurse License
- 2 years LTC facility experience
Qualifications
- Bachelor's Degree in Nursing preferred
- Recent direct patient hospital care or work experience in a Long Term care or Sub Acute and/or TCU setting
Skills
- Strong communication skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office
