Be the voice and guide! You are the key to orchestrating your patients' care across the healthcare continuum. You'll work with physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care experiences and services, improve population health, reduce cost, and achieve collaborative practices that exemplify Main Line Health System's commitment to patient-centered care, community engagement, and population health. Use your excellent communication and advocacy skills to partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care which includes health promotion, disease prevention, and disease management activities.
Responsibilities
- Identify opportunities to improve patient care experiences and services
- Collaborate with physicians, peers, community colleagues, and others to achieve collaborative practices
- Partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care
- Use excellent communication and advocacy skills to advocate for patients and improve their care
- Work with unit councils and evidence-based research projects to contribute to the development of new care models
Requirements
- Three years of recent clinical experience required
- Leadership experience preferred
- Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
- Master's Degree preferred
- Active PA RN license required

Schedule
Per Diem, Varied Shifts; mainly weekends, some weekdays to cover. 16 hours/month + one weekend per month + 1 holiday per year
