Lead with us at MJHS Home Care, where we provide advanced clinical services and emotional support to patients recovering from illness, injury or surgery. Our services are supported by an integrated health system, allowing patients to recover in familiar surroundings.

Responsibilities

• Evaluate the home care needs of patients
• Review patient insurance information
• Cookordinate activities involved in each individual care plan
• Make sure pertinent findings are shared with family and professional caregivers
• Show excellent communication, documentation and time management capabilities
• Experience the respect for your opinions and high level of autonomy needed to do the job right

Requirements

• Excellent clinical assessment skills
• Strong problem-solving abilities and interaction with an integrated team
• Current NYS RN license and registration
• Bachelor’s degree in nursing preferred
• One year acute medical-surgical nursing experience
• CHHA experience preferred
