About the role
The Registered Nurse (RN) Navigator provides patient-centered care by educating, advocating, and coordinating services across the care continuum.
Responsibilities
- Assess patient needs and develop and implement individualized care plans
- Facilitate appointments and access to resources
- Manage symptom control and support treatment adherence
- Serve as a vital link among patients, families, and healthcare providers to ensure effective communication and care coordination
- Develop educational materials and maintain community resource databases
- Assist with referrals and discharge planning
- Participate in research activities and quality improvement initiatives
- Mentor new staff
Requirements
- Active Georgia RN license
- BLS certification
- Minimum of three years of RN clinical experience
Qualifications
Preferred candidates have:
- Experience as a Clinical Nurse Coordinator, Navigator, or Case Manager
- BSN degree
