RN Navigator - Perinatal

Responsibilities

• Identifies patient needs through consultation with physicians, review of records and interviews with the patient and family.
• Works with all services to provide consistent, organized and understandable care for the diagnostic and treatment phases of a patient's treatment plan.
• Collaborates with the patient’s Oncologist to develop an individualized Survivorship Care Plan (SCP).
• Makes appropriate referrals and provides support to and communication with patients and their families regarding the individualized plan of care throughout all phases of the continuum of care up to and including survivorship.
• Ensures that a copy of the treatment summary and SCP are submitted to the patient's primary care physician.

Qualifications

• Bachelor's Degree in Nursing Required
• Master's Degree in Nursing Preferred
• Five (5) years of progressively responsible clinical registered nurse work experience in acute care setting Required
• One (1) year work experience with responsibilities in teaching and managing patients with a disease state related to the specialty area involved Preferred
