RN Navigator Perinatal
About the role
Plans, organizes, and oversees daily clinical and administrative operations of the Perinatal Center. Coordinates high-risk patient care from outpatient referral through hospital admission, discharge, and follow-up. Manages patient care records and care plans. Engages in patient encounters in both ambulatory perinatal units and inpatient settings as needed. Builds relationships with patients and families. Educates patients and significant others on care and treatment options to enable patient-led treatment decisions. Provides informational material to patients. Offers psychosocial and emotional support to patients and families. Coordinates appointments and services for patients. Coordinates care conferences with physicians, staff, and patients/families. Maintains a database on fetal care cases, complex maternal medical cases, and hospital transfers. Provides compassionate, appropriate, and effective patient care.
Responsibilities
- Demonstrates knowledge of the requisite biotechnological and biomedical sciences, as well as application of this knowledge to patient care.
- Demonstrate interpersonal and communication skills that result in the effective exchange of information and collaboration with patients, their families, and health professionals.
- The PNN must demonstrate management approach to common fetal conditions and obstetrical complications requiring specialized antenatal care and/or hospitalization.
- The PNN must demonstrate management approach to maternal medical conditions (e.g., diabetes).
- The psychosocial impact of high-risk pregnancy on the patient and family.
- Develop and manage communication process between interdisciplinary team members, staff, and patients/families.
- Effectively present and discuss patients with all members of the healthcare team, including advanced practice providers, medical students, sonographers, residents, nurses and attending physicians.
- Effectively address patient questions and concerns during each visit.
- Appropriately elevate patient questions and concerns to supervising physician.
- Communicate regularly with referring physicians to provide patient updates and services as needed.
- Demonstrates ability to communicate by reading, writing legibly, speaking, and comprehending English effectively to carry out job requirements.
- Education: Associate's of Nursing [Required], Bachelor's of Nursing [Preferred]. Field Of Study: 1+ experience in OBGYN or Maternal Fetal Medicine [Required], 2+ experience in OBGYN or Maternal Fetal Medicine [Preferred]. Work Experience: 3+ experience in obgyn or mfm [Preferred].
