About the Role
As our MDS / Care Plan Coordinator, you'll be responsible for:
- Coordinating timely and accurate MDS assessments
- Leading interdisciplinary Care Plan meetings
- Monitoring EMR documentation for accuracy and compliance
- Conducting in-services and training on care planning/documentation best practices
- Collaborating with physicians, therapy teams, and department leaders
- Ensuring compliance with state and federal regulations
Requirements
- Current Illinois RN License
- MDS experience
- AANAC certification (Preferred)
- Proficiency in EMR systems and documentation
- Excellent leadership, communication, and organizational skills
