RN MDS Coordinator

About the role

We are seeking a compassionate and experienced RN MDS Coordinator to oversee the coordination and completion of the Resident Assessment Instrument (MDS, CAAs, and Care Plan) in accordance with current Federal and State Regulations at our skilled nursing and rehabilitation facility.

Responsibilities

• Oversee and participate in the coordination and completion of the Resident Assessment Instrument (MDS, CAAs, and Care Plan).
• Monitor and document the management of Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts to continually improve systems and processes.

Requirements

• Current, unencumbered, active state RN license.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six months of experience as an MDS Coordinator.
• RAC-CT or RNAC certification preferred.
