RN, MDS Coordinator
About the role
Coordinates and leads the inputting of Minimum Data Set (MDS) data into the organizational database. Ensures the documentation of departmental admissions and discharges. Ensures all MDS assessment forms are completed for all residents in certified nursing homes. Ensures required assessments for residents, within specific guidelines and time frames, are met. Coordinates small program(s) with limited budget/impact.
Responsibilities
- Sets up and leads the Assessment Reference Data (ARD) and starts the schedule of assessments upon resident admission.
- Captures and ensures processing of ARD's prior to deadline.
- Notifies the Interdisciplinary Team (IDT) of assessment dates.
- Collaborates with the Resident Assessment Coordinator.
- Transmits MDS data on a weekly basis to appropriate agencies.
- Aids in ensuring the Resource Utilization Group (RUG) category information on Medicare residents is forwarded to the business office in a timely manner.
- Aids the Recovery Audit Contractor (RAC) in completing necessary audits.
- Notifies the IDT of patient schedule conferences and the Assessment Reference Data (ARD).
- Provides new admissions with the unit admission packet within 72 hours of admission.
- Places pertinent admission paperwork in the patient record.
- Performs other job-related duties as assigned.
Requirements
- Education and Work Experience: Bachelor's Degree in Nursing (BSN): Preferred
- Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required
