ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conducts required initial, quarterly, annual, and significant change assessments based on Resident Assessment Instruction (RAI) guidelines to ensure compliance with state and federal standards
- Collections, analyzes, and documents data about each resident’s health condition and medical history
- Develops, revises, and reviews person-centered care plans as necessary
- Communicates frequently with residents and their representatives and coordinates care plan meetings
- Collaborates and effectively communicates with nursing staff and facility personnel to coordinate quality care for residents
- Attends Staff/Unit meetings promoting, developing, and fostering a positive working environment
- Functions as an integral part of the Quality Improvement Program and contributes to improving the facility's overall quality of care provided to residents
QUALIFICATIONS
- Associate degree or equivalent from a two-year college or technical school and minimum 2 years of experience in the field
- RN License
- Must have prior experience with MDS assessments and care plans
- Certified as a Resident Assessment Coordinator
- Strong assessment and Clinical skills
- Proficient in electronic health record systems and MDS software
- Effective communication and interpersonal skills
