Responsibilities

• Compiles data, tracks outcomes, and makes recommendations for process improvement.
• Develops and maintains relationships with physicians, post-acute providers, therapists, patients, and families.
• Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services.
• Ensures seamless transitions across the continuum of care.
• Educates patients and families about post-acute care services and expectations.

Requirements

• Associate's of Nursing (Required)
• Bachelor's of Nursing (Preferred)
• 1+ year clinical experience in area of specialization (Required)
• Registered Nurse (RN) (Required)
• Basic Life Support - CPR Cert (BLS) (Required)

Skills

• Competent with Microsoft Office (Required)
• Possess strong organizational skills with the ability to prioritize, manage and complete multiple tasks and projects within specified timeframes. (Required)
• Age specific care for program of focus/clinical area of specialization. (Required)
• Teaching and counseling patients, families and staff as needed. (Required)
• Strong communication skills. (Required)

Pay

Pay Range: $73,902.40 - $130,603.20

Schedule

Full time, Day shift.
