Responsibilities
- Provide skilled nursing care to hospice patients according to individualized care plans and physician orders
- Assess patients' health status, monitor vital signs, and report changes in condition to the RN Case Manager
- Collaborate with physicians, families, and the interdisciplinary care team to coordinate quality patient care
- Obtain and implement physician orders and reorder medications as needed
- Complete accurate and timely electronic documentation, with the majority of documentation completed at the point of care
- Schedule and complete patient visits during regular business hours and participate in weekend/holiday rotation as assigned
- Participate in case conferences, interdisciplinary team meetings, staff meetings, and ongoing educational opportunities
- Maintain compliance with all state and federal home health and hospice regulations
- Perform other duties as assigned
Qualifications
- Current RN or LPN license in the State of Minnesota
- Minimum of 2 years of experience in geriatrics, home care, hospice, or a related healthcare setting
- Strong knowledge of nursing practices and principles
- Excellent assessment, communication, and interpersonal skills
- Ability to establish positive relationships with patients, families, physicians, and interdisciplinary team members
- Strong organizational and documentation skills with proficiency using electronic medical records
- Ability to exercise sound clinical judgment while maintaining compassion and professionalism
- Knowledge of HIPAA guidelines and commitment to maintaining patient confidentiality
