RN/LPN Visit Nurse

Responsibilities

  • Provide skilled nursing care to hospice patients according to individualized care plans and physician orders
  • Assess patients' health status, monitor vital signs, and report changes in condition to the RN Case Manager
  • Collaborate with physicians, families, and the interdisciplinary care team to coordinate quality patient care
  • Obtain and implement physician orders and reorder medications as needed
  • Complete accurate and timely electronic documentation, with the majority of documentation completed at the point of care
  • Schedule and complete patient visits during regular business hours and participate in weekend/holiday rotation as assigned
  • Participate in case conferences, interdisciplinary team meetings, staff meetings, and ongoing educational opportunities
  • Maintain compliance with all state and federal home health and hospice regulations
  • Perform other duties as assigned

Qualifications

  • Current RN or LPN license in the State of Minnesota
  • Minimum of 2 years of experience in geriatrics, home care, hospice, or a related healthcare setting
  • Strong knowledge of nursing practices and principles
  • Excellent assessment, communication, and interpersonal skills
  • Ability to establish positive relationships with patients, families, physicians, and interdisciplinary team members
  • Strong organizational and documentation skills with proficiency using electronic medical records
  • Ability to exercise sound clinical judgment while maintaining compassion and professionalism
  • Knowledge of HIPAA guidelines and commitment to maintaining patient confidentiality
