About the role
Provides hands-on care to each assigned patient and delivers efficient and effective quality patient care.
Responsibilities
- Accepts patient care reports and patient assignments from the RN Team Leader for all patients assigned.
- Collaborates with the RN Team Leader to ensure timely completion of assigned visits and submission of required documentation.
- Coordinates a weekly visiting schedule for all assigned patients and conducts visits according to the schedule.
- Contacts the RN Team Leader on a daily basis for any changes to the schedule.
- Notifies the RN Team Leader of any anticipated problems with the posted schedule.
- Establishes and maintains an effective and efficient visit route and provides direct patient care in compliance with the plan of care.
- Communicates frequently with the RN Team Leader, physicians, other team members, and the patient/family to coordinate changes in patient care and ensure appropriate follow-up and supporting documentation.
- Maintains a complete and accurate calendar book, including disaster codes, emergency information, due dates for lab work, etc.
- Completes assigned patient care visits in accordance with physician orders.
- Documents supporting information accurately and promptly, including visit reports, care plans, medication profiles, progress notes, and discharge summaries.
- Assists in the timely requisition of patient supplies in appropriate quantities.
- Verifies physician licensure.
- Adheres to organizational policies, procedures, and processes, seeking guidance when necessary.
- Participates in evaluations of team member performance and attends inservices and special projects as assigned.
- Maintains confidentiality of business-related information and presents a professional appearance.
- Notifies supervisors of absences or tardiness and maintains a professional attitude.
Requirements
- Licensed Practical Nurse, currently licensed in the state of Tennessee.
- Valid driver’s license.
- Previous experience providing home health visits preferred.
- Working knowledge of the home health care program.
- Computer data entry and word processing skills desirable.
