About the Role

Under the direction of the VP of Post-Acute Care or designee, you will perform diversified duties including communicating with Skilled Nursing Facilities (SNFs) for bed offers for Southcoast Hospitals. You will coordinate with case managers in Southcoast Health hospitals to facilitate patient placements from acute care to SNFs, manage the referral-to-bed-offer process, and oversee information flow regarding patient needs, discharge scheduling, insurance issues, and related concerns.

Your role will involve building strong relationships with SNF accounts to secure bed offers and expedite patient transitions out of acute care. You will also document bed offers, bed flows, and discharge calendars to enhance awareness of care access.

Responsibilities

  • Communicate with Skilled Nursing Facilities (SNFs) to secure bed offers for Southcoast Hospitals.
  • Coordinate with case managers at Southcoast Health hospitals to facilitate patient placements from acute care to SNFs.
  • Manage the entire referral-to-bed-offer process for patient discharges.
  • Oversee information flow related to patient needs, discharge scheduling, insurance issues, and other discharge-related concerns.
  • Build and maintain strong relationships with SNF accounts to maximize bed availability and expedite patient transitions.
  • Document bed offers, bed flows, and discharge calendars to improve access-to-care awareness.

Qualifications

  • Current licensure by the Board of Registration in Nursing (LPN or RN) is required.
  • One to three years of nursing experience is required.
  • At least two years of experience in an SNF/Sub-acute care facility, preferably in an administrative or admissions role.
  • Highly effective interpersonal, customer service, and communication skills.
  • Proficient computer skills, including experience with software, databases, and electronic records programs.
  • Must be fully vaccinated against seasonal influenza and COVID-19 or sign an exemption statement for medical or personal reasons.

Pay

Pay rate will be determined based on level of experience. Pay Range: USD $88,129.60 – USD $161,844.80 per year.
