Job Purpose Obtain post admission authorizationsAct as the liaison between the physicians, hospitals and Insurance Case ManagersAssist in the development of Care Plans for medically complex residentsManage and coordinate caseload upon admission, throughout stay/dischargeTimely communication with updating external physicians/Case Mng Communicate patient care needs with the IDTSupport patients and their familiesAttend weekly Medicare MeetingsEvaluate for denials and assist with appealsCoordinate discharges including transport, meds, and referrals for home care.Follow up with patient and/or family post dischargeOther duties as assigned Position Requirements Three years clinical case management experience with at least one in long term care required.Advanced communication, problem solving and interpersonal skills.Knowledge of Medicare and Medicaid regulations in long term care required.Licensed Nurse or Licensed Social Worker preferred
