About the role
This position will require some on call duties, unit coverage over weekends and/or holidays.
Responsibilities
- Assess patients' needs and develop individualized care plans to promote optimal health and well-being
- Coordinate with healthcare providers, families, and caregivers to ensure seamless transitions and continuity of care
- Monitor patients' progress, make adjustments to care plans as needed, and communicate effectively with all stakeholders
- Provide education and support to patients, families, and caregivers to promote independence and healthy habits
- Collaborate with interdisciplinary teams to develop and implement effective care strategies and protocols
- Document all patient interactions, assessments, and care plans in accordance with facility policies and regulations
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
- Current LPN/RN license in the state of New York
- Prior experience in a healthcare setting, preferably in a case management role
- Strong assessment, communication, and problem-solving skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
- Strong attention to detail and organizational skills
