About the role

Join our compassionate care team at a skilled nursing facility as a Licensed Practical Nurse (LPN) or Registered Nurse (RN). You will play a critical role in providing total nursing care to residents while supervising team members and ensuring compliance with facility standards. Responsibilities

  • Provide total nursing care to residents on the assigned unit during the shift.
  • Participate in developing and implementing individualized care plans alongside the healthcare team.
  • Administer medications and treatments as prescribed and evaluate effectiveness regularly.
  • Maintain an environment that promotes residents' physical, social, and psychological well-being.
  • Educate residents and families on care plans and medical procedures.
  • Monitor team member performance and assist with development, evaluation, and corrective action if needed.
  • Delegate and ensure completion of assigned nursing tasks to team members.
  • Stay current with facility policies, procedures, and ongoing nursing education.
  • Recognize and act on significant changes in resident conditions.
  • Document care, responses, and relevant information per facility guidelines.
  • Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
  • Follow all facility safety policies and procedures.

Requirements

  • Graduation from an accredited nursing education program.
  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Illinois.
  • No prior abuse convictions.
  • Strong communication and teamwork skills.
  • Ability to follow policies, maintain safety, and deliver empathetic care.
  • Must be able to work with diverse resident populations, including elderly and mentally ill individuals.
  • Physically able to perform nursing duties with or without accommodations.
