SaVida Health, a private equity backed healthcare company, provides outpatient opiate and alcohol addiction treatment services. SaVida Health's care model includes medical care, counseling, comprehensive toxicology testing, case management and medical management of psychiatric medications. SaVida is headquartered in Nashville, TN and currently operates in Massachusetts, Delaware, Vermont, New Hampshire, Tennessee, Maine and Virginia, with plans to expand rapidly to meet the needs of patients suffering from opiate and alcohol addiction.

About the Role

A qualified Registered Nurse will offer comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community services for patients with Substance Use Disorder. Medicines prescribed are primarily Buprenorphine and Vivitrol.

Responsibilities

• Comprehensive Care Management: Identify patients for Medication Assisted Therapy, conduct initial assessments with the site team, and help formulate individual plans of care. Manage and improve care across health, substance abuse, mental health treatment, and social service providers.
• Care Coordination: Implement individual plans of care with active patient involvement through linkages, referrals, coordination, and follow-up across treatment and human services settings. Ensure services are coordinated across medical, social, mental health, substance, corrections, educational, and vocational providers.
• Health Promotion: Promote patient activation and empowerment for shared decision-making, support healthy behaviors, and encourage self-management of health, mental health, and substance abuse conditions.
• Comprehensive Transitional Care: Streamline patient transitions between treatment settings, levels of care, and providers to reduce hospital readmissions, facilitate timely community placements, and coordinate treatment information sharing.
• Individual and Family Support: Assist patients in participating in treatment, reduce barriers to care, support age- and gender-appropriate adult role functioning, and promote recovery by helping create and support the patient’s support system.
• Referral to Community and Social Support Services: Assist patients in obtaining and maintaining eligibility for formal supports (e.g., health care, income support, housing, legal services) and connect them to informal community resources.
• Participate in weekly staff meetings, group supervision, clinical and administrative supervision, and Performance & Quality Improvement (PQI) meetings as scheduled.
• Uphold all policies and procedures related to patient engagement in treatment.
• Develop effective working relationships and cooperate with the provider team throughout the case management process.
• Serve as an ambassador for integration between behavioral and medical teams.
• Establish a network of connections with community agencies, hospitals, and service providers to link patients with needed services.
• Provide continuity of care through documentation and communication with collaborative providers and agencies.
• Act as a client advocate to coordinate services or resolve emergency problems in crisis situations.
• Collaborate with therapists, medical providers, and care team members to ensure patient needs are met and monitor program compliance.
• Administer medical injections.
• Maintain proficiency in clinical documentation (assessment, treatment planning, progress notes).

Requirements

• Current RN license in Vermont.
• Demonstrated ability to connect patients to area resources, serve as a patient advocate, and perform case management.
• Excellent interpersonal skills, ability to manage change, and capability to interface with the community collaboratively.
• Commitment to improving the lives of people with Substance Use Disorder and promoting health and disease prevention.
• Ability to work independently and as part of a team.
• Ability to plan and prioritize work with limited supervision.
• Working knowledge of social services, justice, health, community service, and governmental agencies is a plus.
• Ability to serve as a liaison to participants, employees, and professional liability insurance providers, and provide advocacy as needed.
• One year of experience using an Electronic Health Record (EHR); experience with Athena EHR is a plus.
• Computer literacy, including typing skills and knowledge of EHR/EMRs.
• Ability to work with individuals and groups from diverse populations; bilingual/bicultural preferred.
• Excellent verbal and written communication skills.
• Ability to present information in a group setting.
• Strong time management skills to meet deadlines.
