About the role

The Pavilion Rehab & Nursing Center is a Five Star, Joint Commission accredited short‑term rehab, long‑term, and skilled nursing care community located in Hyannis. We are dedicated to our residents and team members and are seeking highly motivated and qualified Nurses to join our nursing department.

Responsibilities

• Assess the health care needs of residents, make a nursing diagnosis, and collaborate with the nursing team to develop an interdisciplinary plan of care.
• Communicate the plan to all care providers to ensure continuity of care.
• Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff, and the community.
• Assess the effect of medications and treatments, monitor effects, and provide appropriate feedback to the interdisciplinary team and medical managers.
• Use the Electronic Health Record (PCC preferred).
• Lead and direct nursing staff in the safe, effective delivery of care by providing structure.
• Provide education to residents regarding healthcare issues and care‑plan strategies.
• Apply knowledge of infection control, fire and disaster policies, and other facility, state, and federal policies to maintain a safe environment.
• Protect resident rights and treat residents, staff, and community with respect and dignity.
• Handle resident and facility property and information with honesty, confidentiality, and respect.

Pay

• Sign‑on bonus: up to $4,000 for RN, up to $3,000 for LPN (24+ hours).
• RN salary range: $41.00 – $53.95 per hour (based on experience).
• LPN salary range: $36.70 – $49.45 per hour (based on experience).
• Weekday shift differential: Nights $5.00 per hour.
• Weekend shift differential: Nights $7.00 per hour.
• RN per diem rate: $47.00 per hour (plus applicable shift differentials).
• LPN per diem rate: $42.00 per hour (plus applicable shift differentials).
