About the role

As an On-Call Nurse with Reserve Care, you will have the autonomy to utilize your nursing skills. You are relied upon to care for patients and families who need help after hours, often during urgent times. You are invited into patients' homes and lives during uncertain times and have the honor of becoming a trusted face. For many of our team members, this is more than a career—it's a calling.

Responsibilities

  • Assess physical, psychosocial, and spiritual needs of the patient and family (RN only).
  • Gather data related to patient and family’s physical, psychosocial, and spiritual needs (LPN).
  • Provide intermittent skilled nursing services to the patient under the patient’s physician’s direction.
  • Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
  • Serve as a liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
  • Provide emotional support to the patient and family.
  • Refer patients and/or family members to other services when appropriate.
  • Provide instruction (health care teaching) to the patient and family as related to care in a residence.
  • Make visit to patient’s residence at the time of death.
  • Maintain, promptly complete, and return all documentation as required.
  • Communicate and act as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Relay pertinent information to appropriate team members.
  • Participate in meetings as initiated by team leader.
  • Participate in mandatory or elective RC University events, trainings, or in-services as necessary.
  • RN’s shall participate in triage coverage as needed.
  • Perform other duties as assigned.

Requirements

  • High School Diploma or equivalent required.
  • Graduate of an accredited school of nursing; Bachelor’s degree preferred.
  • Licensed RN/LPN in the state of Ohio.
  • Experience preferred in pain and symptom management, oncology, or mental health.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work independently and collaboratively with a team.
  • Demonstrated knowledge and skills to provide appropriate care to all age groups.
  • Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite a stressful atmosphere.
  • Ability to access patients and families in any residential and/or public setting in any weather condition (e.g., climbing stairs or snow-filled walkways).
  • Proficiency in Microsoft Office suite of products, particularly Word and Excel, use of shared team files, a donor database, grant portals, and virtual meeting technology.
  • Must have and maintain a valid driver’s license, good driving record, and auto accident insurance at a level acceptable to Reserve Care and/or its insurer.

Physical Requirements

  • Ability to stand and walk for extensive periods of time.
  • Manual and finger dexterity and eye-hand coordination to provide nursing care and operate equipment.
  • This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently, utilizing appropriate safe transfer techniques and body mechanics when working with patients.
  • Ability to stoop, squat, or bend frequently.
  • Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
  • Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, chemicals and/or chemical fumes, pet dander and/or exposure to allergens, and adverse effects of inclement weather, as well as potentially difficult patient and family situations.

Conditions of Employment

  • Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
  • Provide an active driver’s license.
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics, and professionalism.

Schedule

8:00 PM – 8:30 AM, 36 hours a week, every other weekend.
