RN Liaison
Job Summary
RN Liaison Responsible for ensuring patients referred to Evergreen Home Care Services (EHCS) have a smooth transition to Home Care services in the care setting of their choice, educating the community, physicians, and other health care providers about Hospice, Palliative Care, Home Health, and Behavioral Health services.
Responsibilities
- Assesses the appropriateness for Home Care Services for patients in hospitals, skilled nursing facilities, adult family homes and assisted living facilities.
- Interacts and engages daily with hospital staff, skilled nursing facility staff, physician office staff and the Home Care administrative and clinical staff.
- Initiates and maintains collaborative relationships and clear communication between the EHCS management and referral sources.
- Participates in customer service measurements and functions, collecting, analyzing, and sharing information with EHCS management.
- Maintains and grows existing and new relationships with referral sources across the care continuum and throughout EHCS territory.
- Acts as a resource to provide comprehensive solutions to the community and other stakeholders.
Requirements
- Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required)
- Current BLS Healthcare Provider certification by date of hire.
- 2 years of Home Health, Hospice or other Home Care experience.
- Prior marketing, sales and/or community outreach experience.
- Current Washington State Driver’s license and proof of insurability, if the position requires regular driving.
- Working knowledge of Medicare COP, JC Standards, Third party insurance and Medicare HMO’s.

Wage Range: $48.21 - $87.11 per hour
