About the Role

The Cypress Center is looking to fill an opening for a second-shift RN Inpatient Admission Specialist. The position offers a unique schedule of 4 pm to midnight, five evenings per week, with some flexibility to work one weekend evening if preferred. This role will help our designated receiving facility better process referrals from CTC and emergency departments statewide after hours, and work directly with admitting providers to assess medical necessity for a non-hospital-based inpatient level of care.

Responsibilities

  • Receive and review all referrals faxed to the Cypress Center Unit (CCU).
  • Complete prescreening information and arrange referrals for presentation to the doctor.
  • Reach out to referral sources to confirm receipt of faxes, communicate hourly on missing information, and ensure the person still needs a bed.
  • Communicate directly with the Care Traffic Control team at the state hospital and with emergency department nurses/clinicians on bed status and availability.
  • Present all referrals to admitting doctors and advocate for referrals to the unit, including those with dual disorders needing MAT intervention.
  • Complete nurse-to-nurse communications with sending facilities.
  • Handle prior authorizations and concurrent/discharge reviews for insurance when needed or when time allows.
  • Complete eligibilities for patients admitted to CCU over 10 days.
  • Work with referral sources on medication reconciliations and communicate pharmacy needs for new admissions with CCU nurses.
  • Reach out hourly to state referral sources when open beds are available to provide real-time status on unit acuity, open referrals, and missing information.
  • Act as a liaison connecting daytime and evening shifts to pass on information about new admissions and referral status.
  • Participate in phone huddles with local hospitals, CTC, and stakeholders to provide updates on current admissions statuses.
  • Attend Rapid Response/ES shift report huddles to strengthen communication regarding daily bed availability and identify barriers to receiving critical referral information.
  • Perform other duties as required.

Requirements

  • Knowledge of levels of care within the mental health system in NH.
  • Excellent customer service skills.
  • Ability to initiate tasks without delay and advocate for patients with a wide variety of mental health crises, including substance misuse.
  • Solid knowledge of emergency department review of systems and ability to read lab work quickly to identify potential areas of concern in the admission process.
  • Ability to think critically yet positively and remain free of stigma in all communications.
  • Ability to work independently without distraction, as well as part of a multidisciplinary team.

Qualifications

  • RN with experience in a psychiatric acute care environment; may consider an LPN with the right experience.
  • At least 1 year of experience working in a psychiatric setting, particularly in an acute care environment.
  • Active RN or LPN licensure.

Pay

$34.61 - $42.75 per hour

Schedule

4 pm to midnight, five evenings per week (some flexibility to work one weekend evening if preferred).
