About the Shifts & Locations
- Mequon: 10am-2pm, every 4-6 weeks (currently Mondays)
- Beaver Dam: 1:30pm-2:30pm, every 4 weeks (currently Mondays)
- Grafton: 9:30am-11:30am, 2 times a month (currently Wednesdays)
- West Bend: 11:30am-1:30pm, every 4 weeks (currently Wednesdays)
- Iron Ridge: 2:30pm-3:30pm, 1x every other month (currently Wednesdays)

Responsibilities
- Provides care as defined within the scope of the state’s Nurse Practice Act using sound judgment and critical thinking
- If directed by the Director of Nursing, makes the initial evaluation visit and regularly reevaluates the client’s nursing needs
- Initiates the plan of care and makes necessary revisions, keeping the Director of Nursing updated on the revisions
- Provides those services requiring substantial and specialized nursing skill
- Initiates appropriate preventive and rehabilitative nursing procedures
- Prepares clinical and progress notes and summaries of care
- Informs the Director of Nursing, physician and other personnel of changes in the client’s condition and needs
- Counsels the client and family in meeting nursing and related needs
- Assists in the development of organization goals
- Administers agency policies and procedures
- Assists the Director of Nursing with assessing the competency of clinical staff to promote quality, continuity and safety of a client’s care
- Supervises, educates and coaches other nursing personnel, as directed by the Director of Nursing
- Provides information to be used for statistics, reports and records for purposes of evaluation and reporting of organization activities
- Assures proper maintenance of clinical records in compliance with local, state and federal laws
- Assures compliance with all local, state and federal laws
- Identifies and participates in agency performance improvement activities
- Stays informed about changes in the field of nursing and home health care
- Shares information with appropriate organization personnel
- Promotes home health referrals in the health care community
- Providing assistance with daily tasks, including but not limited to: feeding, hoyer lift, bathing, dressing, meal preparation, linen changes, and light housekeeping
Requirements
- Registered nurse with current unencumbered licensure to practice professional nursing in Wisconsin
- Current Peripheral IV starts/maintenance & IV Infusion Therapy
- Injection and self-infusion training
- Phlebotomy for adult and pediatric patients
- Minimum of one (1) year of documented experience preferred
- Possess exceptional nursing assessment skills
- Comply with accepted professional standards of practice
- Excellent interpersonal skills
- Strong clinical skills
- Excellent customer service skills and ability to deal tactfully with clients, staff and community
- Treat clients, staff and the public with courtesy, respect and presents a positive public image
- Case management skills are a plus
- Licensed driver with reliable transportation
- Auto insurance in accordance with the organization’s requirements if transporting clients
- Ensures confidentiality and security of the client’s medical information (HIPAA)
- Ability to work on weekends, nights and holidays is a plus
- Ability to read, write, speak and understand English and communicate effectively
- Understands regulatory compliance
- State requirement for the franchise location
- Federal regulations including but not limited to OSHA, HIPAA
- Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull
- Varied assistance is required depending on the mobility of the client
- Must be able to lift up to 50 pounds
Pay
Infusions pay $77/up to 2 hours for a single visit.
