RN Hospital Community Liaison - Hospice - Full -Time - Washington

About the role

This position entails accepting Hospice, Home Health, and Home Infusion referrals across various healthcare settings, including acute care, clinics, nursing homes, and rehabilitation centers. It involves maintaining an active presence in these environments to provide excellent customer service to discharge planners, social workers, physicians, and patients. The role supports timely discharges and promotes continuity of care within the health system.

Qualifications

• Registered Nurse license required, bachelor's preferred
• Minimum three years of nursing experience, with hospice and home health experience preferred
• Strong assessment, communication, and documentation skills
• Ability to work independently and collaboratively within a team
• Certification in Hospice and Palliative Nursing (CHPN) is a plus
