RN - Hospice Intake Lead

About the role

Responsible for coordination and implementation of the referral process and the coordination and initiation of the admissions process. Works collaboratively with the Nurse Managers to ensure optimal hospice care.

Responsibilities

• Collaborates with Manager on complex referrals
• Assigns pending referrals to referral staff each day
• Assigns portals to specific referral staff each day
• Facilitates afternoon huddle to assess referral activity
• Ensures mission values are translated and integrated into daily work performance and relationships, guided by the values of compassion, dignity, respect, empowerment, and growth
• Utilizes excellent interpersonal skills to promote both external and internal customer satisfaction
• Functions as resource person, mentor, and coach for clinical staff
• Manages resources to provide quality delivery of care within cost-effective parameters and regulatory standards
• Manages phone calls and communications with other members of the health care team
• Assists manager with monthly schedule for referral center staff
• Weekend obligation of one (1) weekend per month as well as one (1) holiday per year

Requirements

• Valid RN license in NJ
• BSN preferred
• 5 years of medical/surgical experience required
• 1 year of hospice/palliative care or three years of oncology experience preferred
• OCN, CRNH, or Board Certification in Nursing Professional Development preferred

Pay

$76,960 - $93,712.51 annually, based on years of experience.
