RN - Hospice - Full-Time Day Shift

About the Role

The registered nurse plans, organizes, and directs hospice care, with an emphasis on community health education and experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.

Responsibilities

• Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness(es).
• Provides professional nursing care utilizing all elements of the nursing process as defined in the state Nurse Practice Act.
• Assesses and evaluates patient’s status by writing and initiating the plan of care, regularly re-evaluating patient and family/caregiver needs, and revising the plan of care as necessary.
• Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals based on nursing diagnosis, incorporating palliative nursing actions.
• Includes the patient and family in the planning process and initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician in the plan of care.
• Counsels the patient and family in meeting nursing and related needs, and provides health care instructions as appropriate.
• Assists the patient with activities of daily living and facilitates efforts toward self-sufficiency and optimal comfort care.
• Acts as Case Manager when assigned by the Clinical Supervisor, coordinating patient care for an assigned caseload.
• Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition and care given, including pain/symptom management changes and outcomes.
• Communicates with the physician regarding patient needs, reports changes in condition, and obtains/receives physician orders as required.
• Coordinates care with community health-related persons and teaches self-care techniques to patients and families.
• Provides medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling.
• Works in concert with the interdisciplinary group to maintain a safe environment and provide continuity of care.
• Meets the emotional needs of the patient and family/caregiver in cooperation with the hospice interdisciplinary group.
• Attends interdisciplinary group meetings and performs other related duties as assigned.
• Participates in on-call duties as defined by the on-call policy and ensures arrangements for necessary equipment and services.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development, maintaining and upgrading professional knowledge and practice skills.
• Fulfills the obligation of requested and/or accepted case assignments and actively participates in quality assessment and performance improvement activities.

Requirements

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care related to hospice.
• Current licensure in the state and CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.
• One year of hospice care experience required.
• Responsible for supervising hospice aides/home care attendants.
• Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
• Prolonged or considerable walking or standing; ability to lift, position, and/or transfer patients; lift supplies and equipment.
• Considerable reaching, stooping, bending, kneeling, and/or crouching.
• Visual acuity and hearing to perform required nursing skills.
• Reliable transportation required.

Work Environment & Physical Requirements

• Requires frequent travel to patient homes and clinical sites.
• Must be able to lift, push, pull, and transport medical equipment and supplies up to 50 lbs.
• Ability to work flexible hours, including occasional evenings and weekends, as required for patient care needs.
• Able to work in a team setting and independently.
• Must be comfortable working with diverse patient populations, including those with chronic illnesses or disabilities.

Qualifications

• Graduate from an accredited professional school of nursing.
• Valid and current RN License in good standing from the Virginia State Board of Nursing.
