Registered Nurse - Gen In Patient Hospice - Full Time - Day Shift

Job Description

The inpatient hospice nurse acts as an end of life specialist and highly skilled resource related to the general inpatient level of care. The RN will take the lead to assess, plan, implement, coordinate, monitor and evaluate options to facilitate the continuum of care through discharge planning. The RN collaborates with the patient or patient’s family/representative, the physician, and members of the multidisciplinary team to facilitate progression-of-care; intervening appropriately to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective.

Education & Experience

• Required: Graduate of an accredited school of nursing
• PREFERRED: Bachelor's of Nursing
• Required: Minimum of 1 year hospital experience
• PREFERRED: Minimum of 1 year hospice experience
